Request Form for a Free Initial Consultation 

Are you interested in possibly hiring me as a Consultant or Mentor? All such relationships start with an initial free half hour consultation (by phone or web conference) to brainstorm your project.

To help prepare both of us for an efficient and productive discussion, please fill out this questionnaire to the best of your ability. Nothing about it is meant to be written in stone. It’s simply to help us focus clearly on what your needs, challenges and purposes are.

After reviewing your responses, I will contact you to schedule our initial consultation. My response time is usually within one week, though some periods are busier than others.

If, after our conversation, we both feel there’s a good fit, we can discuss entering into a contractual relationship.

We will not share your information with third parties, and will use it only to respond to your requests.
